Preparing for you clinical placement...

Please ensure that your academic program has a affiliation agreement in place prior to planning a student placement.  No students may be placed for a clinical experience without an agreement in place. Please review the  ProHealth Care Affiliation Agreement Policy for more information.

 Once an agreement is in place, clinical rotation planning may progress.  The sections below provide information on  placements for Clinical Rotations (multiple students) and Mentorship (one-on-one) experiences.

Clinical rotations:  Rotations with mutiple students

1. Please submit any requests for a clinical placement to Monica Erdmann, Education Program Planner in ProHealth Care's Center for Learning and Innovation.
2. Upon approval of your request, please submit all student information, utilizing the ProHealth Care Student Roster at least 3 weeks prior to your clinical start date.
3. Please schedule iCare Computer Training for nursing students that have not completed a ProHealth Care rotation.  Contact Monica Erdmann to schedule this training session.
4. All students must sign and comply to the ProHealth Care Student Confidentiality Agreement, and understand the information contained in the ProHealth Care New Student Orientation - Online Session.
5. Please schedule a preclinical meeting with the Unit Manager.  The Instructor Orientation Checklist form must also be completed during this meeting. Submit this form to the Center for Learning and Innovation.
6. All students must wear a ProHealth Care Student Indentifcation Badge. Please contact Monica Erdmann to obtain a Badge Request Form.

Mentorship placements:  One-on-one placements

1. Please submit any request for a student mentorship placement to Monica Erdmann.
2. Please submit all student information, utilizing the ProHealth Care Student Mentorship Form at least 30 days prior to your clinical start date.
3. Schedule a iCare Computer Training for nursing students that have not completed a ProHealth Care rotation. Contact Monica Erdmann to schedule this training session.
4. Mentorship students must sign and comply with the ProHealth Care Student Confidentiality Agreement and understand the information contained in the New Student Orientation - Online Session.
5. Please schedule a meeting with the student's mentor, student, and instructor.  The purpose of this meeting is to review the learning objectives of this experience and ensure that questions have been answered.
6. All students must wear a ProHealth Care Student Indentifcation Badge.  Please contact Monica Erdmann to obtain a Badge Request Form.
