Vision

We respond to community health needs by linking individuals most in need to essential health care resources.  This is accomplished through partnerships with existing services and with the community as well as through prevention and health promotion programs and assessment and referral resources.  Keeping the community healthy is our daily mission through partnering with area churches, community agencies, and schools to provide services that help prevent disease and promote better health care decisions.

Parish and Community Outreach Nursing Oconomowoc & Waukesha Memorial Hospitals

The parish nurses and community health outreach nurses are experienced registered nurses who serve as health educators, advisors and advocates for individuals and families in congregations, schools and other community sites.  We help connect people who have health problems or concerns with the community and medical resources they need.  The programs and services we provide differ to meet the unique needs of the people we serve and may include:

  • Helping overcome obstacles to getting the health services you need, regardless of your age or financial resources.
  • Coordinating health related services and advocating for you, when necessary.
  • Answering questions or concerns about your health or the health of a family member.
  • Identifying your own health challenges and referring you to the right resources.
  • Providing important health assessments and health screenings, such as blood pressure or weight.
  • Reinforcing teaching and knowledge of diseases and their treatments.
  • Creating and sustaining a healthy culture for our communities.

Coordination of Care

When you work with a Parish Nurse or Community Outreach Nurse at either Oconomowoc or Waukesha Memorial Hospitals, your rights are protected as if you were a patient.  This means you can:

  • Expect appropriate and respectful care that considers your values and beliefs and be treated with dignity.
  • Participate as much as possible in planning your care
  • Be informed in terms you can understand.
  • Have an assessment of any pain you might experience.
  • Complete a Living Will or Health Care Power of Attorney document.
  • Expect your medical information to be kept confidential and private.
  • Know that there is a hospital policy and procedure for responding to client complaints.

To honor you with the best possible service, our Parish and Community Outreach Nurses ask that you:

  1. Cooperate in your own health care.
  2. Provide accurate information.
